Change of Address

Please notify our staff as soon as possible of ANY changes, especially your telephone number and address as we may need to contact you for urgent medical matters. You will be asked to complete a form for change of address. To notify a change of name, please ask advice at Reception.

Cancelling your Appointment
If you are unable to attend an appointment with one of the doctors or nurses, please telephone or use the link at the bottom of this page to cancel your appointment.

Test Results
Enquiries about results should be made after 12:30pm. In order tomaintain confidentiality, laboratory results will only be given directly to the patient or carer.

Home Visits
Home visits are for patients who are housebound and for those who are too ill and cannot attend the surgery. Please call the surgery before 11.00am if you wish to request a home visit. Please give a brief description of the symptoms and give a contact telephone number.
